Job Summary: We are seeking a highly motivated and organized Care Manager to join our team. In this role, you will coordinate patient care, collaborate with healthcare professionals, and ensure optimal patient outcomes. This position requires strong communication, organizational, and problem-solving skills, as well as a passion for patient-centred care.
Responsibilities:
- Patient Care Coordination: Collaborate with the client, their family, agency team, and other healthcare professionals to develop and implement comprehensive patient care plans. Monitor client progress, identify potential problems, and adjust care plans as needed. Provide education and support to clients and their families regarding their health conditions and care plans.
- Communication and Collaboration: Maintain clear and effective communication with clients, families, and agency staff. Facilitate communication and collaboration among different departments and disciplines. Participate in team meetings and rounds to ensure coordinated care.
- Documentation and Record Keeping: Maintain accurate and up-to-date client records and documentation. Ensure compliance with relevant regulations and standards.
- Resource Management: Identify and access appropriate resources for clients and their families. Advocate for client needs and ensure access to necessary services.
- Other Duties as Assigned: Perform other duties as assigned by the Agency Administrator and Agency Director.
Qualifications:
- At least two years of experience in care management, healthcare, or home care settings is preferred.
- Licensed Vocational Nurse or Social Worker.
Skills:
- Strong communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a team.
- Proficiency in electronic health record (EHR) systems.
- Knowledge of relevant healthcare regulations and standards.
- Empathy, compassion, and a patient-centred approach.
Compensation: $45,000.00 - $55,000.00 per year.
