We are looking for a compassionate Resident Care Coordinator to join our team! The Resident Care Coordinator assists the Resident Care Director and Mind & Memory Care Director in the successful operation of the resident care department, including hiring, training, scheduling, and participating in all resident care associate disciplinary actions. This position ensures departmental compliance and serves as a member of the community management team.

Responsibilities

• Ensure compliance with all resident care and state regulations (e.g., medication management, introductory visits, skills checks, in-services, service plans, etc.)
• Monitor overtime usage
• Schedule resident care staff for Assisted Living and Memory Care
• Assist in developing ongoing care plans based on residents\’ physical and psychological needs
• Develop service plans according to state regulations
• Assess, communicate, and document residents’ status accurately
• Communicate care staff levels and needs, including Lead Aide and Medication Aide needs
• Observe and report significant changes in resident behavior and health
• Complete follow-up processes for incidents and accidents
• Handle follow-up issues with residents and families effectively
• Co-Facilitate RCA meetings

Requirements

• Must be a Certified Nursing Assistant or Home Health Aide with valid Massachusetts certification
• Minimum of two years’ experience working with elders, with supervisory and management skills
• Valid driver’s license
• Ability to move at least 50 lbs. without assistance
