In this position, you will be responsible for providing tailored guidance and support to individuals aged 16 to 25 who are preparing to leave the care system. As a Personal Advisor, you will work collaboratively with social workers, education providers, and other agencies to ensure that each young person receives the comprehensive support they need to thrive. Responsibilities
- Provide individual support and guidance to young people in the Leaving Care Service.

- Facilitate access to housing, education, and vocational training opportunities.
- Collaborate with social workers, educational institutions, and other relevant agencies.
- Monitor and evaluate the progress of young people and adjust support plans as necessary.

Requirements
- A relevant degree in social work, youth work, or a related field.
