Overview
Homelessness Project Workers (SC5) to join our team in the Health, Wellness and Fitness industry. The role supports individuals facing homelessness by providing essential services and resources to enhance well-being and facilitate pathways to stable housing. The position involves collaborating with a diverse team, building relationships with clients, guiding them through complex systems, and empowering them to take charge of their lives. Responsibilities
- Provide direct support and guidance to individuals experiencing homelessness
- Conduct assessments to determine the needs and goals of clients
- Develop and implement personalized action plans for clients

- Build and maintain relationships with community organizations and service providers
- Assist clients in overcoming barriers to housing stability and personal well-being
- Document client progress and maintain accurate records in compliance with organizational policies
Requirements
- Bachelor\\\’s degree in social work, psychology, or a related field
- Experience working with vulnerable populations, particularly individuals experiencing homelessness
- Strong communication and interpersonal skills

- Cultural competency and sensitivity to diverse backgrounds
- Knowledge of community resources and housing services
- Proficiency in Microsoft Office Suite and case management software
