Responsibilities
- Conduct comprehensive assessments to identify clients\\\’ needs and strengths.
- Develop, implement, and monitor individualized support plans in collaboration with clients and their families.
- Provide direct support and guidance to clients in navigating social services and accessing available resources.
- Facilitate referrals to appropriate services including health care, mental health support, and housing assistance.
- Advocate on behalf of clients to ensure their rights and preferences are respected in all service areas.
- Engage with community organizations and stakeholders to promote inclusive services and resources for adults in need.
- Document client progress and maintain accurate records in compliance with regulatory standards.
Requirements
- A relevant degree in Social Work or related field and valid social work qualification.
- Demonstrated experience working with adults in a social care or community setting.
- Strong understanding of social care legislation, policies, and best practices.
- Excellent communication and interpersonal skills for effective client engagement.
- Ability to handle sensitive situations with empathy, discretion, and tact.
