Responsibilities:- Conduct comprehensive assessments to determine the needs of clients and their families.
- Develop and implement individualized intervention plans in collaboration with clients.
- Provide counseling and support to clients in coping with various life challenges.
- Advocate for clients' access to community resources and services.
- Facilitate support groups and educational workshops for clients and their families.
- Maintain accurate and confidential client records and documentation as required by law and organizational policies.
- Collaborate with other professionals, including healthcare providers, to ensure integrated care for clients.
- Master's degree in Social Work from an accredited institution.
- Current state licensure as a Social Worker.
- Minimum of two years of experience in a social work setting.
- Strong knowledge of community resources and support services available to clients.
- Excellent communication and interpersonal skills, enabling effective interaction with clients and colleagues.
- Demonstrated ability to work independently and manage a caseload effectively.
- Commitment to professional development and staying current with best practices in the field of social work.
