We are seeking a dedicated and compassionate Out of Hours Support Worker G5 to join our dynamic team. This role is vital in providing support and assistance to individuals during non-traditional hours, ensuring they receive the care and attention they need when traditional services are unavailable. As an Out of Hours Support Worker, you will play a crucial role in promoting the well-being and independence of those in our care, thereby making a positive difference in their lives.
Your responsibilities will involve:
- Providing direct support to individuals during out of hours periods.
- Responding effectively to emergencies and crisis situations, ensuring safety and well-being.
- Assisting individuals with daily living activities, promoting independence and dignity.
- Maintaining accurate records of support provided and incidents that occur during shifts.
- Collaborating with other team members and professionals to ensure coordinated care.
- Communicating effectively with individuals, families, and relevant stakeholders.
- Supporting the development and implementation of care plans tailored to individual needs.
Requirements:
- Previous experience in support work or a related field is preferred.
