Beluga Rox is recruiting a Care Assessor for their client to support the delivery of safe, person-centred care across Chester in a role that’s varied, meaningful, and truly people-focused.

The Role

  • You’ll complete assessments and reviews, develop person-centred care plans, and keep accurate records using digital systems.
  • You’ll also support care staff and liaise with families and healthcare professionals to ensure clients receive the right care at the right time.

Key Responsibilities

  • Carry out new client assessments and regular care reviews
  • Create and update care plans that reflect individual needs
  • Maintain accurate records using digital care systems
  • Support carers and communicate effectively with clients, families and professionals
  • Take part in the on-call rota and provide emergency support when required

  • NVQ Level 3 in Health & Social Care (or above)
