Good Afternoon, I am currently representing Enfield Council, who are offering an initial temporary contract for a period of 3-6 months with a view to be extended or offered a permanent position for the right candidate at a rate which is negotiable dependent upon experience.
We are looking for a Specialist SEN & Disability Outreach Worker. The right candidate will:
- Work as a specialist SEN and Disability outreach worker, within Enfield's Service for Disabled Children.
- Build effective partnerships with parents/carers, professionals, and service providers to ensure positive outcomes.
- Support children and young people and their families to access effective and person-centred short breaks and community activities.
- Provide advice, information and guidance to children and young people with SEN and disabilities and their families.
- Undertake short break assessments and reviews to ensure that the right support is provided and continues to meet identified needs.
- Work alongside the SEND Transition Manager in ensuring that transition pathways are progressed in a timely way and contribute to Moving On events.
- Ensure that the participation and views of children and young people with SEN and Disabilities inform service development.
- Undertake the lead professional role for children and young people with SEND and disabilities when appropriate.
- Work alongside Social Workers to ensure that the support plan is progressed.
