We are currently recruiting for an experienced Occupational Therapist to join the Haverhill Area Integrated Team within Adult Social Care on behalf of Shropshire Council. This is an excellent opportunity for a skilled OT looking for a flexible hybrid role within a supportive integrated team environment.
Key Responsibilities:- Managing a varied Adult Social Care OT caseload
- Completing face-to-face functional assessments within the community
- Supporting adults to remain independent in their own homes
- Assessing for equipment, adaptations, moving & handling solutions and reablement support
- Completing recommendations for minor and major adaptations
- Working closely with health professionals, social care teams and partner agencies
- Providing specialist OT advice and support to colleagues and trusted assessors
- Supporting urgent moving & handling cases and complex needs assessments
- Adult Social Care Occupational Therapy experience
- Strong assessment and case management skills
- Experience with equipment, adaptations and reablement planning
- Good knowledge of Care Act legislation and OT practice
- Community-based OT experience preferred
- HCPC Registered Occupational Therapist
