Somerset Council is seeking a dedicated and passionate Social Worker to join a supportive multi-disciplinary team based in Shepton Mallet. This is an excellent opportunity for a Social Work England registered professional who is committed to delivering positive outcomes for vulnerable adults and working collaboratively with partner agencies.
Key Responsibilities
- Manage a varied caseload within Adult Social Care.
- Undertake strengths-based assessments and care planning.
- Work collaboratively within a multi-disciplinary team environment.
- Complete safeguarding enquiries and risk assessments.
- Develop effective support plans to promote independence and wellbeing.
- Liaise with Health Services, Education, Voluntary Organisations and partner agencies.
- Maintain accurate case records and reports using electronic systems.
- Ensure practice remains compliant with relevant legislation, policies and Social Work England standards.
Essential Requirements
- Qualified Social Worker.
- Social Work England registration.
- Experience working within Adult Social Care, Learning Disabilities, Mental Health or Community Teams.
- Strong understanding of Care Act legislation and safeguarding principles.
- Experience working within multi-disciplinary teams.
- Excellent assessment, communication and report-writing skills.
- Ability to build effective relationships with service users, families and partner agencies.
- Strong IT and case management system skills.
- Fluent English language skills as required under the Immigration Act 2016.
Desirable Experience
- Learning Disabilities experience.
- Supervisory or mentoring experience.
- Knowledge of local authority social care services.
- Understanding of current government guidance and legislative developments.
- Experience working with complex cases and safeguarding concerns.
