Belmont Recruitment are currently looking for a Social Care Assessor to join Cheshire East Council on an initial 3-month temporary contract based at Leighton Hospital. This is a full-time role working 37 hours per week, Monday to Friday.
Key Duties and Responsibilities
- Carry out needs-based assessments with vulnerable adults and carers, identifying risks, priorities, and appropriate responses.
- Develop, implement, coordinate, and review individual care plans.
- Act as Care Manager, ensuring services meet assessed needs in a timely and cost‐effective way.
- Monitor and review care plans, responding to changes in circumstances.
- Liaise and negotiate with health professionals, statutory and voluntary agencies, families, and carers.
- Provide information on the financial implications of care, undertake financial assessments, and signpost to benefits advice where appropriate.
- Maintain accurate and up-to-date electronic and written records.
- Work flexibly across the borough as required to meet service demands.
Essential Requirements
- Experience working with vulnerable adults and carers in a social care or health setting.
- Knowledge of needs-based assessment and care management processes.
- Understanding of safeguarding, risk assessment, and relevant legislation.
