Rehabilitation Officer
Initial temporary contract for 3 to 6 months with potential to extend or offer permanent employment for the successful candidate. Salary will be negotiated based on experience.
Responsibilities
- Use advanced skills, knowledge and understanding of visual‑impaired specialism to assist in the development of service provision and promote independence, choice and control for service users.
- Carry out duties within a quality and equalities framework based on Council policy, performance targets, professional standards and best practice.
- Conduct specialist assessments of need for adults and children with visual impairments, including complex cases with additional disabilities, in accordance with council policies and legislation (NHS and Community Care Act 1990).
- Support service users, families and carers with specialist advice, prescribing training, equipment and services; assess appropriate support that meets their rehabilitation needs – including mobility training, daily living skills, self‑advocacy and risk evaluation.
- Promote and support personalisation through the Direct Payments process (RAS, Quickheart, Indicative Budget).
- Develop outcome‑focused support plans with the service user that identify and manage risk, ensuring agreed rehabilitation goals preserve or improve their independence.
