Belmont Recruitment are currently looking for a Care Coordinator to join Cheshire East Council on an initial 3-6 month temporary contract. This is a full-time role working 37 hours Monday to Friday in a Hospital Setting. Key Responsibilities * Completing needs-based assessments and identifying individual needs, risks and appropriate support. * Coordinating and reviewing Care Plans and managing a caseload of vulnerable people. * Working closely with clients, carers, relatives, professionals and statutory and voluntary agencies. * Completing financial assessments and advising clients and carers on the financial implications of care services. * Maintaining accurate and up-to-date client records and information across relevant systems. About You We are looking for someone with experience working with vulnerable people and their carers within a social care or health setting. Experience working across different social care settings and with other agencies would be beneficial. You should have knowledge of needs-based assessments, care management, risk assessment and safeguarding procedures, as well as an understanding of relevant legislation, policies and care standards. An NVQ Level 4 or equivalent experience in Social Care is required, with NVQ Level 3 or equivalent experience also recognised within the specification.
