Belmont Recruitment are currently looking for a Care Coordinator to join Walsall Council s Adult Social Care Team on an initial 3 month temporary contract. This is a full-time role working 37 hours per week, Monday to Friday. Key Responsibilities * Undertake assessments of vulnerable adults and contribute to support planning in line with Adult Social Care legislation and best practice. * Carry out allocated care management duties, ensuring assessments and reviews are completed to a high standard. * Develop person-centred support plans that promote independence, choice, dignity and wellbeing. * Work closely with service users, families and carers, ensuring they are fully involved in the assessment and care planning process. * Promote self-directed support, enabling individuals to access appropriate community resources and services. * Support individuals in maintaining their health, wellbeing, choice and control, with full regard to mental capacity. * Maintain accurate records and contribute to the continuous development of the team and service. Essential Requirements * Experience working within Adult Social Care, Care Coordination or Care Management. * Knowledge of person-centred approaches, safeguarding and relevant Adult Social Care legislation. * Experience carrying out assessments, reviews and support planning for vulnerable adults.
