Job Details:
- To provide personal care, emotional and practical support to people with identified care and support needs within a Registered Care Home setting.
- This includes older people, people with a diagnosis of dementia and/or adults with a learning or physical disability within an environment that enables supports and maintains a homely atmosphere ensuring that the rights, dignity, diversity, independence, and wellbeing of individuals are promoted and maintained, with the flexibility to work additional hours across other homes.
- To ensure the principles of a meaningful day are implemented and sustained whereby individuals are supported to participate in activities which enable direction and purpose to their daily lives.
- To ensure that meaningful activities are offered that meet individuals' physical, sensory, cognitive, emotional and social needs and capabilities, based on an individual's Life History Profile, and whereby proactive support, advocacy, monitoring, and evaluation of activities is undertaken.
- To support and enable individuals to undertake all tasks of daily living skills (i.e. washing, dressing, administration of medication), as identified in an individual's Personal Plan, at all times promoting ability and independence.
- To support individuals to access their local and wider community; this may include activities, medical or other appointments.
- To undertake duties of the Care Home in full compliance with Regulation and Inspection of Social Care (Wales) Act 2016.
- To undertake duties at the home in full compliance of service and Corporate Policies and Procedures.
- To contribute and participate in a range of services to meet the needs of individuals.
- To participate in quality assurance systems in the service.
- To support individuals and representatives to contribute to, monitor and review individual personal plans and to support individuals to achieve personal outcomes identified within them.
- To contribute to care management and inter-agency reviews of individual needs.
- To evaluate the risk of abuse, failure to protect, and harm to self and others and to promote and undertake responsibilities within Wales Safeguarding Procedures.
- To participate in own supervision and professional development requirements.
- Registration with Social Care Wales.
- To maintain continuous professional development of self and identify training requirements and participate in any training deemed appropriate to the roles and responsibilities of the role in line with departmental training strategies.
- To contribute to the maintenance and safety of the building and its contents.
- To undertake any required general domestic duties within the Registered Care Home in line with identified tasks and schedules relevant to that Care Home.
- To liaise and work effectively with a wide variety of multi-agency persons, both internally and externally who support individuals.
- To demonstrate the willingness and ability to work within any Registered Care Home within the Directorate.
- To undertake any other duties as directed by the Director of Social Services which may be deemed legal and reasonable at the time of instruction.
