We are looking for compassionate, reliable, and experienced Support Workers to join our team supporting adults aged 18 and over with Learning Disabilities (LD) and Mental Health (MH) needs.

About the Role You will provide person-centred support that promotes independence, choice, and wellbeing.
The individual you will support is very independent and does not require personal care or physical interventions.
Your responsibilities will include: * Supporting independence and daily living skills. * Encouraging community engagement and social activities. * Providing emotional support and promoting positive mental wellbeing. * Assisting with appointments, shopping, and recreational activities where required. * Maintaining accurate daily records and communicating effectively with the wider team. * Promoting dignity, respect, and safeguarding at all times.
