We are seeking dedicated and compassionate Support Workers to join a specialist team providing support to a young woman in her own home.
About the Role
You will work as part of a consistent and dedicated team supporting a young woman with Prader-Willi Syndrome and Autism. She enjoys animals, maintaining an active lifestyle and participating in activities within her local community. Your role will be to support her to achieve her personal goals, develop her independence and live a fulfilling life on her own terms. Working closely with colleagues, family members and other professionals, you will provide person-centred support that is responsive to her individual needs, aspirations and preferences.
Key Responsibilities
- Providing high-quality, person-centred support in accordance with individual support plans
- Supporting activities of daily living whilst promoting independence and choice
- Assisting with budgeting, personal care where required, and the development of life skills
- Encouraging participation in social, recreational and community-based activities
- Providing emotional support and developing positive, professional relationships
- Working collaboratively with families and multidisciplinary professionals
- Supporting the individual to make informed decisions and achieve personal outcomes
- Supporting individuals whose behaviour may challenge in a positive, proactive and person-centred way
- Maintaining accurate records and adhering to organisational policies and procedures
About You
