GENERAL INFORMATION
The Belfast Health Care Trust
The Belfast Health and Social Care Trust was established in April 2007, comprising the Belfast City
Hospital, Mater Hospital, Musgrave Park Hospital, Royal Hospitals and South and East Belfast and
North and West Belfast Health and Social Services Trusts.
Hospital Profiles
Belfast City Hospital (BCH) is a major teaching hospital, most of which is housed within the Tower.
The Northern Ireland Cancer Centre opened on the campus in March 2006. A strong strategic focus on
molecular medicine, cancer and renal services has enabled the development of a vigorous research
programme, together with a large Cancer Clinical Trials Unit. There is a modern radiology department
and substantial AHP Services (e.g., Physiotherapy, Podiatry, Occupational Therapy, Speech &
Language Therapy, Nutrition & Dietetics, etc) and a comprehensive range of pathology.
The Royal Hospitals is the largest hospital complex in Northern Ireland, comprising the Royal Victoria
Hospital, the Royal Jubilee Maternity Hospital (RJMH), the Royal Belfast Hospital for Sick Children
(RBHSC) and the School of Dentistry. It provides virtually all referral services in Northern Ireland and
undoubtedly the vast majority of local research. Major re-developments of the Royal Group of Hospitals
are underway, which includes the Royal Belfast Hospital for Sick Children. A new Critical Care Building
with emergency department and operating theatres is currently in the process of opening. The Royal
Hospitals play a major role in clinical education, training and research, with most academic departments
linked to the Queen’s University Medical School on the Royal Hospital’s complex – medicine, surgery,
ophthalmology, child health, obstetrics and gynaecology, and pathology.
The Mater Hospital is a long established general hospital with teaching status affiliated to the
Queen’s University of Belfast. A state of the art ward block, the McAuley Building, was opened in
January 2002. The X-Ray Department is sited in the Dempsey Building, which was opened in 1991.
The Dempsey Building also houses the Accident & Emergency Department, the Operating Theatres, the
Intensive Care/High Dependency Unit and the Outpatient Department. The main Psychiatric Unit
occupies a separate building next to the Dempsey Building. There is also a Psychiatric Day Hospital on
a separate site, approximately one mile from the main hospital.
Musgrave Park Hospital is the Regional Orthopaedic Unit for Northern Ireland. The Musgrave Park
Regional Orthopaedic Service is the largest in the British Isles with 48 consultant orthopaedic surgeons
and staff. On site is the Queen’s University of Belfast’s Department of Orthopaedic Surgery which is the
largest academic unit in Orthopaedics in the British Isles with an international reputation and an
extensive research output

specialties in Northern Ireland and also serves as the local District General Hospital (DGH). The
post holder will support the delivery of general paediatric nephrology services in RBHSC.
Northern Ireland has a population of 1.85m and an annual birth rate of 23000 per year. RBHSC records
about 35000 attendances at its Emergency Department (ED). RBHSC is an 91 bed paediatric hospital
(12 PICU) which functions as a District General Paediatric Unit and in addition houses most of the
Paediatric Regional Specialities for Northern Ireland, including Intensive Care, Neonatal Surgery,
Trauma and Orthopaedics, Plastic Surgery and Burns, Child Psychiatry, Nephrology, Neurology,
Neurosurgery, Cardiology, Respiratory Paediatrics, Infectious Diseases, Cystic Fibrosis, Haematology,
Oncology, Rheumatology, Gastroenterology, Inherited Metabolic Disorders, Diabetology, Endocrinology
and Dentistry.
The main hospital laboratory complex, including the Regional Endocrine Laboratory and Virus Reference
Laboratory, is a short distance away on site. Within RBHSC there is a Radiology Department, a Dietetic
Department, Physiotherapy Department, Clinical Psychology, Speech Therapy, Occupational Therapy
and Play Specialist Department. A new building, opened in 1999, provides significantly upgraded
accommodation for A&E Department, Outpatient Department, Theatres and Paediatric Intensive Care
Unit as well as the Medical Records Department. The regional neonatology unit is located within the
Royal Jubilee Maternity Service on the Royal Hospitals site. The outline business case for a new
Children’s Hospital has just been accepted and this will be physically adjacent to the new maternity
hospital and neonatology.
Paediatric Nephrology Service
The Paediatric Renal Unit is one of 13 designated comprehensive centres for Paediatric Nephrology in
the United Kingdom. It provides a comprehensive diagnostic and treatment service for children with
renal disorders. The population served is 1.85 million. The Unit is situated in a purpose built annexe to
Barbour Surgical Ward. There are four designated renal in-patient beds and three haemodialysis
outpatient stations.

The service is also supported by interventional radiology, who offer percutaneous diagnostic renal
biopsies twice weekly. A monthly renal pathology meeting takes place at Belfast City Hospital.
The post holder will hold one general nephrology clinic and one specialist clinic per week and see
regional tertiary and local secondary referrals. The case-mix includes relatively common conditions such
as congenital anomalies of the kidney and urinary tract (which are referred both locally and regionally)
It also includes less common conditions such as:
- Glomerulopathies
- Tubulopathies
- Renovascular disease
- multisystem diseases with renal involvement (for example systemic lupus erythematosusmitochondrial abnormalities and metabolic abnormalities )
- complex hypertension
Specialist clinics cohort children by their clinical diagnosis. These clinics include:
- Complex glomerulopathy including congenital nephrotic syndrome
- Complex tubulopathy
- Low clearance CKD clinic
- Dialysis
- Renal transplant
- Renal calculi
The post holder will be expected to establish and develop a renal calculi clinic in liaison with Paediatric
Urology in order to develop a combined nephro-urology clinic. The aim is to run this clinic four times per
annum.
Outreach
Outreach clinics will be established in each of the five district general hospitals in order to offer
education and support delivery of care to children with nephrological conditions in a local setting. The
post holder will be expected to develop links and establish a clinic at one of five District General
hospitals with an aim to deliver an outreach clinic 4 times per year.
Acute Renal Failure
The post holder will be responsible for the tertiary level treatment of patients with acute renal failure.
This will involve the management of oligoanuria and the provision of acute haemodialysis and peritoneal
dialysis with conditions such as haemolytic uraemic syndrome, acute tubular necrosis and tubular
interstitial nephritis.
Intermittent dialysis support will also be provided to other specialities including the paediatric intensive
care unit, paediatric haematology/oncology and the tertiary neonatal unit of the adjacent Royal Jubilee

The post holder will be one of 3 consultants assigned to the chronic kidney disease and renal replacement therapy outpatient service. The post holder will deliver 24 multidisciplinary clinics per year for the management of patients with CKD stage 3b-5. A CKD multidisciplinary meeting will occur the following day to discuss patients reviewed at this clinic.
A monthly psychosocial multidisciplinary meeting will also take place on the 1 st Tuesday of every month along with a multidisciplinary CKD review and monthly planning meeting for all children with CKD stage 3b-5. All members of the MDT are expected to attend both of these meetings.
Transplantation
There is a weekly transplant clinic. A transplant multidisciplinary meeting will be held the following day to discuss all patients reviewed. The post holder will not be expected to deliver either of these services due to their commitment to the CKD/renal replacement therapy service.
The post holder will be expected to attend a monthly multidisciplinary transplant/review meeting to discuss any upcoming transplant recipients and be appraised of any issues in those already transplanted.
Transplant surgery is performed by the Transplant Surgeons from Belfast City Hospital but the pre- operative and entire post-operative management is delivered by the Paediatric Nephrologist.
The post holder will look after children during and after transplantation as part of their service week.
Many end stage renal failure children will also need to be seen on a daily basis in the time immediately following renal transplantation or if they develop complications. These patients are frequently seen as ward attenders. We have established a close working relationship with the adult nephrology/transplant services in the Belfast City Hospital. Regular transplant multidisciplinary team meeting are held with respect to transplant listing. The post holder will be expected to attend a quarterly Nephro- Urology Transplant (NUT) meeting to aid preparation and planning for all renal transplant recipients with urological issues.
Support to other Paediatric Specialties
The post holder will advise other Paediatricians within RBHSC and at a tertiary level in Northern Ireland about complex electrolyte problems or other renal issues. Dialysis support is often required for conditions not involving renal failure such as severe metabolic acidosis. Antenatal consultations for serious renal disease are provided on an ad hoc basis following referral from the tertiary perinatal clinic with plan to develop fetal clinics for antenatal counselling.
General Paediatrics
The post holder will provide support and advice to other Paediatricians within RBHSC and in the District General Hospitals on renal related queries. Dialysis support will be offered if needed for conditions not involving renal failure such as severe metabolic acidosis.
