A new exciting opportunity for an experienced social worker to join our innovative Maximising Independence Team has arisen. You will work as part of a multi-disciplinary team, supporting people with complex and enduring mental health challenges who are assisted by adult social care.
The Maximising Independence Team aims to help individuals become more independent by reassessing those currently supported in the Adult Mental Health Social Care Service and assisting them in moving on from long-term provision through a multi-disciplinary team approach to regain abilities and skills, set enablement goals, and take advantage of opportunities to grow and develop their lives.
Your role will involve collaborating with other social workers, social care practitioners, occupational therapists, and support workers to deliver key activities with targeted groups, including:
- Young people transitioning from children's services into adulthood
- Individuals in residential placements who could benefit from supported living opportunities
- Supporting people in supported living settings to move to private tenancies with appropriate care and support arrangements
You will need a good understanding of the Care Act 2014, the Mental Capacity Act 2005, and the Mental Health Act. Knowledge, skills, and experience in supporting individuals with complex mental health challenges to transition from intensive service delivery to less supported settings would be desirable.

If appointed, your starting salary will be at the bottom of the grade, but if you provide payslip evidence of a higher basic pay, we will consider starting you at a point of minimum advantage within the grade. Equality of pay is extremely important to us.
