The role involves working in the Care Transfer Hub and on a rota covering evenings and weekends; it is office based and focused on discharge planning for hospital inpatients.
You will work within a multidisciplinary environment, supporting safeguarding, Care Act and Mental Capacity Act compliance, and providing flexible support.
