Are you a motivated and dedicated nurse with Chronic Disease skills/qualifications? Do you have experience in Diabetes, Asthma & COPD? If so, we would be interested to hear from you. We are seeking a professionally dedicated Chronic Disease nurse in the Practice to contribute, as necessary, to the wider Practice Nursing Team effort in the support of patients. The role would be to join an existing nursing team of specialist nurses in asthma, COPD and diabetes supported by a MDT team of paramedics, nurse practitioners and GP. We can be flexible in the job requirements in order to accommodate the right candidate offering between 16 - 24 hours a week.
Main duties of the job:
- Assist in chronic disease management of our Asthma, COPD & Diabetes patients including, where necessary, referring patients to secondary/specialist care.
- Implement and evaluate individual treatment plans for chronic disease patients that promote health and well-being.
- Identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients' health.
- Contribute to the overall effort of the Practice nurse team.
- Request and process pathology services as necessary.
- Maintain accurate clinical records in conjunction with extant legislation.
- Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
- Support the clinical team with all safeguarding matters.
- Work effectively as a team and autonomously to ensure patient care is maintained and upheld in accordance with guidance, protocol and contracts.
Job responsibilities:
The successful candidate will be professional, motivated and able to work collaboratively and autonomously as part of our Practice multidisciplinary team. On successful appointment, you will ensure nursing services are delivered effectively to the entitled patient population for chronic illness management in line with guidelines, protocols and contractual agreements. The role will entail a clinic for telephone and face-to-face appointments to assess, review and follow up in patient care. You would be required to undertake the review of pathology if required and liaise with internal and external staff to ensure the patient receives the right care first time. The successful candidate will, depending on skills and experience, take on chronic illness management of the practice patients in the reviewing and delivery of clinical policy and procedure.
Qualifications:
- Registered Nurse (Nursing and Midwifery Council)
- Experience of chronic disease management (specifically with experience of the management of Asthma/COPD/Diabetes).
