Social Worker – Adults (Hospital Discharge)

Monday to Friday | 09:00 – 17:00
Up to £48.39/hr (Umbrella)
Overview
We are looking for a dynamic and experienced Social Worker to join our Hospital Discharge Team, supporting adults with timely, safe discharges and person-centred care planning.
Key Responsibilities
- Carry out assessments & reviews under the Care Act
- Be present on-site at the hospital when required
- Attend Decision Support Tool (DST) meetings
- Support with hospital discharges and multi-agency working
- Work closely with health professionals & families
Requirements
- Qualified Social Worker (SWE registered)
- Minimum 2 years of hospital social work experience
- Sound understanding of the Care Act 2014
- Knowledge of NHS Continuing Healthcare & DST
- Enhanced DBS (adults)
