We are seeking a compassionate, reliable and motivated Community Support Worker to work in the Adult Services Contact Team.
Qualifications
You will be educated to NVQ Level 3 in Social Care or hold a Diploma in Health and Social Care.
Responsibilities
- Answer telephone calls and provide advice as well as signposting to community resources.
- Complete Care Act assessments and support people in a strength based way.
- Enable people to access opportunities that will improve their wellbeing and quality of life.
- Provide advice and signposting to help individuals remain in their local community with support.
