We are seeking a compassionate, reliable and motivated Community Support Worker to work in the Adult Services Contact Team.

Qualifications

You will be educated to NVQ Level 3 in Social Care or hold a Diploma in Health and Social Care.

Responsibilities

  • Answer telephone calls and provide advice as well as signposting to community resources.
  • Complete Care Act assessments and support people in a strength based way.
  • Enable people to access opportunities that will improve their wellbeing and quality of life.
  • Provide advice and signposting to help individuals remain in their local community with support.
