We are seeking a motivated Reablement Team Leader to join our team. This role is focused on supporting individuals to regain independence and achieve the best possible outcomes in their own homes.

Key Responsibilities:

  • Plan and manage staff rotas to ensure effective service delivery.
  • Assess customers' needs and complete comprehensive risk assessments.
  • Identify and arrange appropriate equipment and support to help customers regain and maintain independence.
  • Monitor customer records and documentation, ensuring accuracy and compliance.
  • Review care packages regularly and identify when amendments or additional support may be required.
  • Conduct staff supervision meetings and Personal Development Reviews.
  • Carry out observations of staff practice, providing coaching, guidance, and constructive feedback.
  • Promote high standards of care, safeguarding, and person-centred support at all times.
  • Work collaboratively with customers, families, healthcare professionals, and other stakeholders to achieve positive outcomes.

Experience:

  • Experience working within reablement, home care, or adult social care services.
  • Strong leadership, organisational, and communication skills.
  • Knowledge of risk assessment processes and person-centred care planning.
