We are seeking a skilled professional to join our team as a Future Navigator. This is an exciting opportunity to make a positive impact in the lives of care-experienced young people aged 16–25, supporting them through their journey to independence. The role involves helping young people develop essential life skills and emotional resilience. You will work collaboratively with social workers until they turn 18, then guide them through adulthood. Your primary focus will be on providing tailored support that empowers individuals to achieve their goals.
Key Responsibilities:
- Developing and implementing individualized plans for each client
- Maintaining accurate records and reports
- Collaborating with social services teams and other stakeholders
- Providing ongoing support and guidance throughout the transition process
