We are expanding our Hospice at Home service and are seeking compassionate, committed people to join our growing team. This role offers the opportunity to support patients and families in their own homes as the service extends across North Nottinghamshire.
The Hospice at Home service provides care for palliative patients seven days a week in their own home.
This role will be based in Bassetlaw with travel across North Nottinghamshire to meet organisational needs. The specific base locations within this area have yet to be confirmed., As a Registered Nurse within the Hospice at Home service, you will deliver high-quality, person-centred nursing care to patients in their own homes across North Nottinghamshire. You will support patients with palliative and end of life care needs and provide appropriate support to families and carers, in line with hospice policies and agreed care plans.
The role includes assessment, planning, and delivery of nursing care, symptom management, care of the dying, and psychosocial and spiritual support. You will promote independence where possible, support discharge home, and respond flexibly to changing patient needs.
You will work as an autonomous practitioner while remaining part of a supportive multidisciplinary team. You will work closely with the Hospice at Home Lead, Health Care Assistants, Care Navigators, the Integrated Hub, and wider system partners within the End-of-Life Care Together Alliance.
You will contribute to maintaining high standards of care, safety, and quality, ensuring practice aligns with NMC standards, CQC requirements, and the values and philosophy of Beaumond House Hospice Care., We are contactable 24 hours a day, 7 days per week, to provide a friendly ear, general information or to signpost patients, relatives and carers to appropriate resources and services.

Ensuring our communities receive the care and comfort they need, free of charge, when every moment matters. Our Care is provided in people\βs homes across our communities and at the Hospice in Newark.

Beaumond House Hospice Care is a registered charity. We provide four In-patient beds, Day Therapy, and Hospice at Home Care. Hospice at Home supports people with palliative and end of life care needs, often in the last weeks and months of life. Our nurses and health care assistants provide assessment, personal care, emotional support, advice, and advance care planning in the persons preferred place of care.
