Beaumond House Hospice Care delivers personalised, high-quality end of life care across hospice, home, and community settings. Patients and families remain at the centre of everything we do. All staff are expected to uphold dignity, safety, confidentiality, and the highest standards of physical, psychological, and spiritual care. This role sits within the End-of-Life Care Together Alliance and operates across organisational boundaries to deliver safe, coordinated, and person-centred care for people in North Nottinghamshire.

Main duties of the job:

  • To work within the End-of-Life Care Together Hub to provide coordinated, compassionate support to patients in the last year of life, including those in their final days.
  • The role focuses on ensuring patients, carers and professionals can access the right services at the right time, supporting admission avoidance and timely discharge planning in line with individual Advance Care Plans.
  • Through effective care coordination, communication, administration and data management, the Care Navigator supports the work of clinical teams across the End-of-Life Care Together Alliance, while upholding the values, standards and ethos of Beaumond House Hospice Care.

We treat every person with respect. We deliver compassionate, professional care that supports physical, psychological, and spiritual needs. We look for staff and volunteers who protect dignity, safety, and confidentiality. Beaumond House Hospice Care is a registered charity. We provide four In-patient beds, Day Therapy, and Hospice at Home Care. Hospice at Home supports people with palliative and end of life care needs, often in the last weeks and months of life. Our nurses and health care assistants provide assessment, personal care, emotional support, advice, and advance care planning in the person's preferred place of care. We are contactable 24 hours a day, 7 days per week, to provide a friendly ear, general information or to signpost patients, relatives and carers to appropriate resources and services.

Job responsibilities:

  • Care Navigation and Patient Support: Act as a first point of contact for patients, carers and professionals contacting the End-of-Life Care Together Hub. Provide clear information about services and support options and help people navigate these. Support patients and families to understand and follow their Advance Care Plan and preferred place of care. Offer ongoing practical and emotional support, recognising changing needs and escalating to clinical staff when required. Work in partnership with primary and secondary care colleagues, members of the End-of-Life Care Together Alliance, the Beaumond House team and other support roles.
  • Referral Management and MDT Coordination: Receive and process incoming referrals for palliative and end of life care in line with agreed protocols. Check referral information for completeness and seek further information where needed. Triage and direct referrals to the appropriate clinician or service within the Alliance. Arrange, attend and minute End of Life Care Together Hub MDT meetings. Prepare MDT agendas and caseload lists so all relevant health and social care professionals can attend and contribute. Record agreed actions from MDTs and follow up on these within agreed timeframes.
  • Information Management and Administration: Record all relevant contacts and interventions in electronic systems, including SystmOne, e-HealthScope, Nervecentre and other agreed systems. Maintain accurate, timely and complete records in line with policies and data protection requirements. Support data collection, reporting and evaluation for the End-of-Life Care Together Hub, including outcomes for new roles or pathways. Develop and maintain simple data collection tools to support audit and service evaluation. Undertake general office duties, including preparation of patient notes, scanning and document management.
  • Partnership Working and Service Pathways: Work closely with hospice services, GPs, community nursing, acute wards, social care and voluntary sector partners. Help to coordinate care for agreed pathways so patients receive appropriate care at each stage of their illness. Highlight delays or barriers in the pathway and work with colleagues to reduce their impact on patients and families. Participate in internal and external working groups and projects linked to end-of-life care. Support the development and delivery of collective projects across general practice and Alliance partners.
  • Service Development and Improvement: Contribute to the ongoing development of the End-of-Life Care Together Hub. Support strategy and workstream planning by providing information, feedback and practical project support. Help implement the Comprehensive Model for Personalised Care, completing required training. Contribute to evaluation reports that describe activity, outcomes and service effectiveness.
  • General Duties: Work collaboratively with all Beaumond House and Alliance colleagues. Maintain confidentiality at all times and comply with the Data Protection Act 2018. Follow all health and safety policies and procedures and take reasonable care for your own and others safety. Attend and complete all mandatory training, including safeguarding adults and children. Adhere to Equality and Diversity policies and support inclusive practice. Use relevant IT systems needed for the role. Use resources responsibly and highlight ideas for more efficient use of equipment and supplies. Provide cross cover within the Care Navigation function as required. Work flexibly to meet the needs of the service, including working at other sites if needed. Carry out any other duties within the band and scope of the role as requested by your line manager.
  • Professional Development and Performance: Take responsibility for identifying your learning needs and discuss these at appraisal. Take part in supervision and reflective practice to support your wellbeing and effectiveness. Participate in appraisal and agree personal objectives that support Beaumond House priorities. Support and mentor colleagues where appropriate within your competence and role.
  • Professional Standards: You will work within Beaumond House and End of Life Care Together Hub policies, procedures and agreed pathways. You will plan your daily workload in line with service priorities, asking for guidance from your line manager when this is needed. You will keep your knowledge and skills up to date and take part in learning and development relevant to the role.

£15.46 an hour. An additional £1 per hour for weekends.
