We are expanding our Hospice at Home service and are seeking compassionate, committed people to join our growing team. This role offers the opportunity to support patients and families in their own homes as the service extends across North Nottinghamshire. The Hospice at Home service provides care for palliative patients seven days a week in their own home. This role will be based in Ashfield with travel across North Nottinghamshire to meet organisational needs. The specific base locations within this area have yet to be confirmed.
Main duties of the job:
- As a Registered Nurse within the Hospice at Home service, you will deliver high-quality, person-centred nursing care to patients in their own homes across North Nottinghamshire.
- You will support patients with palliative and end of life care needs and provide appropriate support to families and carers, in line with hospice policies and agreed care plans.
- The role includes assessment, planning, and delivery of nursing care, symptom management, care of the dying, and psychosocial and spiritual support.
- You will promote independence where possible, support discharge home, and respond flexibly to changing patient needs.
- You will work as an autonomous practitioner while remaining part of a supportive multidisciplinary team.
- You will work closely with the Hospice at Home Lead, Health Care Assistants, Care Navigators, the Integrated Hub, and wider system partners within the End-of-Life Care Together Alliance.
- You will contribute to maintaining high standards of care, safety, and quality, ensuring practice aligns with NMC standards, CQC requirements, and the values and philosophy of Beaumond House Hospice Care.

We treat every person with respect. We deliver compassionate, professional care that supports physical, psychological, and spiritual needs. We look for staff and volunteers who protect dignity, safety, and confidentiality.
Beaumont House Hospice Care is a registered charity. We provide four In-patient beds, Day Therapy, and Hospice at Home Care. Hospice at Home supports people with palliative and end of life care needs, often in the last weeks and months of life. Our nurses and health care assistants provide assessment, personal care, emotional support, advice, and advance care planning in the person's preferred place of care.
We are contactable 24 hours a day, 7 days per week, to provide a friendly ear, general information or to signpost patients, relatives and carers to appropriate resources and services.
Job responsibilities:
- Work in partnership with the Integrated Hub, Hospice at Home Lead and Care Navigators.
- Support discharge planning and reduce avoidable hospital admissions.
- Communicate effectively with patients, carers and professionals.
- Attend and contribute to MDT, GSF and Hub meetings.
- Build strong working relationships with partner organisations.
- Lead and support Health Care Assistants in delivery of care.
- Delegate duties appropriately in line with NMC guidance.
- Act as role model, supervisor and mentor.
- Contribute to recruitment, induction and staff development.
- Monitor staffing levels and elevate concerns as required.
- Support students and learners in practice.
- Act in accordance with safeguarding adults policies.
- Undertake risk assessments including lone working.
- Maintain accurate documentation in line with NMC and CQC standards.
- Comply with medicines management policies.
- Participate in audits, incident reporting and quality improvement.
- Provide bereavement support to carers and families.
- Identify and refer those requiring specialist bereavement services.
- Maintain NMC registration and comply with the NMC Code.
- Engage in reflective practice, supervision and appraisal.
- Attend mandatory and developmental training.
- Build resilience and support emotional wellbeing.
Skills, Experience and Attributes Required:
- Experience in caring for palliative patients or relevant transferrable experience.
- Good telephone and interpersonal communication skills.
- Ability to work as part of a team and respect for all individuals.
- Flexibility, excellent listening skills and ability to build rapport with others, with a friendly and sensitive approach.
- Confidence in use of basic IT (Microsoft 365) and electronic records (SystmOne) or willingness to develop this.
- Full driving licence and regular access to a suitable vehicle.
- Ability to communicate effectively in spoken and written English to support safe care, accurate record keeping, and sensitive communication with patients and families.
General Duties:
- Work collaboratively and cooperatively with Beaumond House colleagues and End of Life Care Together Alliance partners.
- Work in accordance with hospice values of compassion, dignity, respect and excellence, recognising the emotional impact of end-of-life care.
- Respect confidentiality in all areas of Beaumond House.
- Comply with the Health and Safety at Work Act.
- Work within Beaumond House policies and procedures.
- Participate in and contribute to team meetings and Hub meetings as required.
- Always behave in a professional manner, reflecting and maintaining Beaumond House values.
