We have an exciting opportunity for a Specialist Practice Nurse to join our nursing team at Beacon Medical Group for 17 hours per week.
Main duties of the job
As a key member of the nursing team, working within the guidelines of the UKCC Code of Professional Practice and the Scope of Professional Conduct, you will be providing and maintaining a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. In addition, you will be providing and maintaining a high standard of chronic disease management for patients with chronic diseases, which includes the diagnosis, monitoring and development of individual management plans, and agreeing these as appropriate with the patient and other health professionals.
Job responsibilities
You will be working as a Specialist Practice Nurse providing a high level of care for patients in accordance with the guidelines of the UKCC Code of Professional Practice and the Scope of Professional Conduct. This is a varied role and will be based across our six GP practices.
Experience
- Competent in basic nursing duties required for the post.
- Experience of working in accordance with the NMC Code of Professional Conduct.
- Experience of managing and monitoring the care of Chronic Disease Patients.
- Experience of dealing with the public and patients in a healthcare setting.
- Experience of Primary Care/General Practice.
- Experience of System1 clinical system.

- Assessor/Supervisor Qualification.
