Job responsibilities include:

  • Undertaking medicines reconciliation in accordance with practice protocols and updating patients' medical records/prescribing accordingly.
  • Carrying out medication reviews while in patient records, e.g. dose optimisation and medicines synchronisation.
  • Supporting the prescription clerks by helping with queries relating to prescription requests and looking for ways to improve the efficiency and safety of the prescription issuing process.
  • Interfacing with community and hospital pharmacy colleagues on medicines-related issues.
  • Implementing and supporting the use of monitored dosage systems and supporting the district nursing team with medicine optimisation for housebound patients.
  • Implementing and supporting electronic prescribing and repeat dispensing in appropriate patients.
  • Reviewing out-patient letters which include changes to drug treatment and making changes to patients' repeat medication lists as appropriate.
  • Liaising with patients and community pharmacy regarding changes.
  • Reviewing out-patient discharge recommendations and issuing prescriptions as requested.
  • Supporting the achievement of the practice's prescribing Quality and Outcomes Framework targets.
  • Working with the Lead Clinical Pharmacist, Pharmacists, and Clinicians in accordance with agreed practice prescribing.
  • Contributing to the review and development of repeat prescribing protocols/Standard Operating Procedures.
  • Driving quality improvements in areas of own workload that will improve Population Health management.
  • Providing advice to GPs, staff, and patients in changes to prescribing.
  • Participating in medication audits and ensuring findings are recorded and shared with the wider team.
  • Assisting in the appropriate monitoring and management of prescribing budgets.
  • Preparing evidence-based resources and information to support the implementation of rational cost-effective prescribing.
  • Providing supervision for the wider team to ensure staff are carrying out their duties appropriately.
  • Supporting the team to ensure dispensing standards are met and relevant audits are completed.
  • Supporting Clinical Pharmacists to provide medication reviews and patient information leaflets.
  • Taking a proactive approach to the safe handling of prescriptions.
  • Responding to medication queries from patients and staff in a professional manner.
  • Advising on the sourcing and safe management of medicines.
  • Using highly developed knowledge and skills to provide high standards of patient-centred care.
  • Working with the Care Home Pharmacist to offer Pharmacy Technician support.
  • Undertaking patient-facing and patient-supporting roles to ensure effective medicines use.
  • Providing medication review services to patients via clinics, domiciliary visits, and in residential and nursing homes.
  • Assisting the Lead Clinical Pharmacist with data analysis of prescribing behaviour.
  • Supporting the safe and effective repeat prescribing of high-risk medicines.
  • Documenting interventions made to patients' medications in their health care records.
  • Maintaining confidentiality, security, and integrity of information relating to patients, staff, and the business.
  • Positively supporting the equality, diversity, and rights of patients, carers, and colleagues.
  • Assisting in promoting and maintaining health, safety, and security.
  • Recognising the importance of effective communication within the team.
  • Striving to maintain and improve quality within the practice.
  • Applying practice policies, standards, and guidance.
