  • Location: Various vacancies due to promotions across Bournemouth, Christchurch and Poole
  • Salary: £44,075 for Level 2 or £48,226 for Level 3, depending on experience

What You'll Do

You will support people living with long‑term conditions to live well in the community. You will work alongside them, their carers and families to understand what matters most and what support is needed to help them live a safe and independent life.
