Community Based Adult Social Care Practitioner – Intake. Our team works within the Adult Social Care Hub to support residents over 18 and their carers. We focus on enabling people to minimise dependence and maximise independence through strength‑based, self‑directed support.

What You Can Expect To Be Doing
- Conduct comprehensive assessments of adults and carers to determine needs and promote social welfare and inclusion.
- Provide direct support in homes, care homes and hospital settings.
