BCHA is collaborating with Dorset Healthcare, The Lantern, and Help and Care to deliver wellbeing hubs across Dorset. These hubs are part of the universal offer of wellbeing support, an element of the mental health transformation programme for adults. The hubs aim to support individuals in taking meaningful steps by connecting them with necessary resources at the right time and place, empowering them to make their own choices and fostering full citizenship within their communities. Citizenship here involves having a voice, a sense of belonging, purpose, identity, and equal access to rights and opportunities.

Position:

At our hub, you\’ll create a welcoming environment where people feel safe and valued. You\’ll build rapport and adopt a strengths-based approach, recognizing individuals as experts on themselves. This role involves enabling choice, advocating for individuals in multi-disciplinary meetings, and knowing when to escalate concerns.

You\’ll promote local resources and well-being opportunities, helping people advocate for themselves. You\’ll develop personalized learning and well-being plans, focusing on safety and utilizing community resources. Support will be provided through various communication methods, ensuring confidentiality and effective care planning.

By addressing health inequalities and engaging communities, you\’ll build essential partnerships with local professionals. Accurate record-keeping and caseload management are crucial. You\’ll encourage active citizenship, engage with communities, and commit to ongoing learning and reflective practice. Join us to make a real difference in people\’s lives!

Requirements:
