As a Support Worker, you will play a key role in preventing homelessness and supporting people to achieve lasting positive change. You'll build trusted relationships with customers, providing practical and emotional support that enables them to overcome barriers, maintain accommodation, and work towards independent living. This is a varied and rewarding role where no two days are the same. You'll work closely with internal teams and partner agencies to deliver coordinated, person‑centred support tailored to each individual's goals and circumstances.
Key Responsibilities
- Provide high‑quality, person‑centred support to adults experiencing homelessness or at risk of homelessness, including those with complex needs.
- Manage a caseload of customers, completing assessments and developing personalised support plans focused on achieving sustainable outcomes.
- Complete and maintain support plans, risk assessments, safeguarding records, and case notes using the INFORM system.
- Support customers to access and sustain suitable accommodation, including social housing applications, supported housing referrals, and private rented options where appropriate.
- Help customers maximise their income by supporting benefit claims, budgeting, debt management, and financial inclusion.
- Work collaboratively with health professionals, social care, probation, substance misuse services, mental health teams, and other partner agencies to ensure coordinated support.
- Advocate on behalf of customers, assisting with appointments, applications, grant funding, and accessing essential services.
- Identify safeguarding concerns and make referrals to Adult Social Care and other appropriate agencies where required.
- Encourage customers to develop independent living skills, improve wellbeing, build social connections, and work towards personal goals.
