BAYADA Home Health Care is seeking an RN Case Manager for our Reading, PA Adult and Pediatric Skilled Nursing office. This office provides top-quality care to adult and pediatric clients throughout Reading, PA and surrounding areas.

Responsibilities of the RN Case Manager:
- Lead, inspire, and motivate staff to provide exceptional care.
- Conduct assessments at client homes in Reading, PA, and surrounding areas.
- Develop and oversee care plans.
- Supervise LPNs, RNs, and home health aides in the field.
- Facilitate new hire orientations and conduct competency assessments for staff.
Qualifications:
- Registered Nurse (RN) with at least 1 year of experience.
- Supervisory experience preferred.
