Bay Cove Human Services’ mission is to partner with people to overcome challenges and realize personal potential. Bay Cove pursues this mission by providing individualized and compassionate services to people facing the challenges associated with developmental disabilities, mental illness, substance use disorders, and homelessness at more than 175 program sites throughout Greater Boston and Southeastern Massachusetts.

Job Summary:

Engages program participants in regular case management services, focusing on steps needed to retain stable, permanent housing. Provides comprehensive, intensive, case management support to chronically homeless people who identify as female with histories of substance use and/or mental illness, domestic violence, participation in transactional or survival sex, or experiences with commercial sexual exploitations to obtain and maintain permanent supportive housing using a Housing First model. The Case Manager uses a harm-reduction, trauma-informed approach in working with participants centered around non judgment, acceptance, dignity and respect. The Case Manager is responsible for a caseload of 6 tenants, in permanent housing in the community who identify as female.

Additional responsibilities include, but are not limited to: assisting tenants with obtaining benefits, healthcare, educational opportunities, employment opportunities, and legal aid; supporting the development of tenants’ independent living skills and creating individual service plans with tenants; complying with the maintenance of required client record documentation; monitoring of and reporting maintenance, or health and safety needs in apartments.

This is a non-exempt position.

Essential Job Duties and Responsibilities:

  • Engage program participants in case management services focused on retention of permanent stable housing.
  • Complete intake and assessment interviews to determine participant needs upon referral to the Sponsor based, permanent supportive housing program. Work with outreach specialists throughout the intake process.
  • Work with tenants to create client-centered, strengths-based, individual service plans (ISP), to be periodically evaluated and edited as necessary/appropriate, but at least once every 90 days.
  • Actively manage a case load of at least 6 formerly female gender identified homeless tenants in SRO housing promoting housing retention, income maximization, and greater self-determination. Provide high quality, culturally sensitive, client-centered intensive housing based case management utilizing evidence-based practices of motivational interviewing, trauma-informed care and harm reduction. Using creativity and perseverance when necessary to engage the most difficult to reach tenants.
  • Educate, coach, and reinforce independent living skills (ILS), including, but not limited to personal hygiene, budgeting, cooking and leisure time activities. Assist tenants in shopping, laundry, and other ILSs as needed or requested.
  • Provide strengths-based interventions with tenants experiencing psychiatric distress or other acute crisis. Develop individually based crisis intervention plans with each tenant. Intervene directly and/or cooperate with team efforts around crisis stabilization for tenants.
  • Maintain accurate written records and documentation in accordance with program standards including, but not limited to ISPs, tenant charts, incident reports, detailed case notes, referrals, and BSAS Intake Documents.
  • Complete VESTA records in a timely and accurate fashion and contribute to Annual Progress Reporting when needed.
  • Provide assistance and advocacy for participants working to secure benefits such as SSI/SSDI, Veteran Disability, SNAP/EAEDC, unemployment, health insurance, and other related services.

Requirements for the position:

  • The person in this position must have a clear commitment to the population we serve, and be able to work as part of a team.
  • BA/BS preferred (degree requirement waived with additional experience)
  • Two years case management and/or direct care experience in areas such as homeless services/housing search, substance use treatment, mental health, and/or harm reduction services
  • Proficiency in Microsoft Office (Windows) and Google Apps
  • Demonstrated professional communication and organizational skills
  • Experience working with women with extensive trauma histories strongly preferred.
  • Bilingual Spanish/English speaker preferred

  • Ability to work effectively independently and within an interdepartmental team.
  • Sensitivity to the needs of adults with mental illness, substance use and/or developmental disabilities.
  • Commitment to improving the quality of services available to people with disabilities
