Bay Cove pursues this mission by providing individualized and compassionate services to people facing the challenges associated with developmental disabilities, mental illness, substance use disorders, and homelessness at more than 175 program sites throughout Greater Boston and Southeastern Massachusetts.
Job Summary:
The ATARP program serves adults with co-occurring mental health and addictions disorders using a \”housing first\” approach. The Outreach Worker will be responsible for direct service, case management and required paperwork for program participants.
This is a non-exempt position.
Supervisory Responsibilities:
None
Essential Functions of Position:
- Develop meaningful, professional relationships with assigned caseload using recovery principles, trauma informed practice and Motivational Interviewing techniques.
- Provide direct support, care coordination, and case management for assigned individuals including assistance with housing, relapse prevention plans, activities that promote recovery, appointments, and other community living supports.
- Assist individuals in apartment set-up and maintenance, including shopping and assistance on move in day.
- Provide on-going support for housed individuals to retain tenancy in good standing and integrate positively into the community.
- Complete documentation pertaining to the electronic health record, including housing applications, service notes, outcome measures, and other documentation assigned in a timely manner.
- Provide crisis assessment, prevention, and needed intervention when appropriate.
- Provide advocacy and maintain effective communications with liaison services with the appropriate agencies and resources regarding appointments, recommended treatment, engagement, housing, substance use, etc.
- Maintain healthy landlord relationships, assist individuals report maintenance issues, and work with landlords to resolve DMH Licensing Deficiencies.
- Complete other duties and projects as assigned by the supervisor.
Qualifications:
- BA preferred (degree requirement can be waived with an additional year of experience).
- Two years direct care experience providing services to adults with co-occurring mental health and/or substance abuse concerns
- Valid driver\’s license, use of personal vehicle required for job tasks (with mileage compensation)
- Proficiency in word processing and other computer functions.
- Demonstrated professional communication and organizational skills.
- Flexibility to work some evenings and weekends on an as needed basis.

- Demonstrated ability to work effectively and collaboratively with a culturally diverse population of clients, other care providers, family and community members.
