About the Team: Our Adult Social Care Hospital Team works closely with local community hospitals across the area, delivering short term assessment, supporting the Discharge to Assess (D2A) pathway and responding to safeguarding needs. Following team expansion and a recent retirement, we are pleased to offer 3 Social Worker vacancies within this supportive and collaborative team. You will join a skilled team including a Service Manager, Team Manager, Social Workers, Social Care Practitioners and administrative staff, all working together to achieve safe, timely discharges and positive outcomes. You will also work in close partnership with our Community Pathways Team, supporting individuals discharged home with reablement, ensuring ongoing recovery, review and safeguarding where required.

About the Role: This role focuses on delivering high quality, strengths based social work to adults and carers, supporting people through safe and timely discharge from hospital via the Discharge to Assess model, alongside completing the initial review in the community. You will support individuals to be assessed in the most appropriate setting, including community hospitals, residential/nursing placements and their own home, enabling better outcomes and promoting independence while ensuring risk is effectively managed. Safeguarding is a key part of this role, ensuring individuals are protected, supported and empowered through a multi‑agency approach.

Responsibilities:

  • Undertake strengths-based Care Act assessments with adults and carers
  • Support and deliver the Discharge to Assess (D2A) process, ensuring timely discharges and assessment in community settings
  • Lead and contribute to safeguarding enquiries (Care Act s.42), applying Making Safeguarding Personal
  • Develop and review person centred support plans promoting independence and wellbeing
  • Carry out Mental Capacity Assessments and Best Interest decisions
  • Manage an allocated caseload, ensuring timely progression and flow through the system
  • Maintain high quality case recording, safeguarding documentation, and reports
  • Work collaboratively with health colleagues, families, housing, and voluntary sector partners
  • Promote early intervention, prevention, and access to community resources
