Our practice is values-driven and evidence-based with a strong ethos of empowerment and independence. Join our Adult Care Locality Team in North East Somerset, based in Midsomer Norton! You will work in a dedicated, friendly, and supportive team including a Service Lead, Team Manager, Senior Practitioner, Social Workers, Social Care Practitioners, and administrative staff.
About the role
To deliver high-quality, strengths-based social work with adults and carers within a locality community team. The postholder will undertake proportionate assessments, support planning and review, safeguard adults in line with B&NES procedures, and promote wellbeing, independence, and inclusion. The role supports B&NES’s vision for personalised, preventative, and community-focused care, working in partnership with individuals, families, and multi-agency colleagues.
Key Responsibilities
- Undertake strengths-based Care Act assessments with adults and carers, identifying needs, risks, outcomes, and community assets to inform proportionate intervention.
- Lead or contribute to Care Act s.42 safeguarding enquiries, applying Making Safeguarding Personal, multi-agency collaboration, and defensible decision-making.
- Develop, implement, and review person-centred support plans that promote wellbeing, independence, and least-restrictive options, ensuring timely progression of work.
- Carry out Mental Capacity Act assessments and Best Interest decisions, supporting DoLS processes in line with legal requirements and B&NES practice guidance.
- Hold and manage an allocated caseload, using professional judgement to prioritise work, produce clear, outcome-focused plans and ensure cases move forward without delay.
- Complete high-quality case recording, risk assessments, and professional reports in accordance with B&NES standards, policies, and statutory guidance.
- Work collaboratively with carers, families, health colleagues, housing, community organisations, and voluntary sector partners to support holistic, integrated planning.
- Provide professional advice, early intervention, prevention, and advocacy, enabling access to community resources and local support to reduce or delay formal care needs.
