About the Team

As part of our expanding Adult Social Care workforce, we have a one new permanent Assessment and Review Officer post available within the North East Somerset Locality Team.

About the Role

We are looking for highly motivated individuals to work alongside Social Workers, using strengths‑based approaches, to help us deliver our statutory functions in a person‑centred, proportionate, and enabling way.

Responsibilities

  • Utilise a strengths‑based model of practice, undertake assessments, and develop and review care and support plans to ensure they meet shared/best outcomes.
  • Multi‑agency working in close partnership with customers, carers and other stakeholders.
  • Support Social Work Teams in delivering their statutory duties under Care Act (2014).
  • The role will focus on our statutory Care Act 2014 review duties, carers assessments and supporting drop‑in sessions for residents who may be in need of care and support.
