We are seeking a dedicated and passionate Key Worker to join our Alternative Education Team. You will provide intensive support to young people aged 16-25 with Special Educational Needs and Disabilities (SEND), helping them access educational opportunities and prepare for adulthood. This role involves developing personalised action plans, coordinating with professionals, and supporting young people in their educational and personal development journeys. This is a community-based position and there will be a requirement to travel to young people\βs homes across B&NES, and you will need to be able to support young people to travel to sessions.
Responsibilities
- Provide intensive support to young people with SEND.
- Develop personalised action plans focusing on strengths, needs, and aspirations.
- Support access to volunteering, work experience, and educational opportunities.
- Act as the primary point of contact for young people and their parents/carers.
- Coordinate with a team of professionals to deliver holistic support.
- Attend and contribute to multi-agency meetings.
- Ensure high attendance rates and timely completion of assessments and action plans.
- Engage in 1-to-1 activities tailored to young people\βs needs, including vocational, PSHE, and enrichment activities.
- Assist in exploring next steps beyond the programme, ensuring smooth transitions.
- Maintain accurate records and ensure compliance with safeguarding and data protection protocols.
