Overview

In this role you support children and young people navigating CAMHS services, embedding within the CAMHS team to provide non-clinical, person-centered guidance. You will build trusted relationships, coordinate access to support, and advocate for equity of access across diverse groups. The role focuses on social prescribing and collaborative care to improve experiences and outcomes for those entering, receiving, or exiting CAMHS. You will work with cross-functional partners to shape holistic support at a community level.

Responsibilities

  • Establish and maintain relationships with children, young people, families and external agencies to enable timely access to support
  • Maintain a directory of local community groups to enable effective social prescribing
  • Identify support needs and goals with young people and contribute to initial assessments
  • Hold and manage a caseload providing social prescribing and follow-up support
  • Advocate for the rights and needs of young people within the CAMHS system
  • Guide families on mental health pathways and available services
  • Collaborate with CAMHS and OTR teams to coordinate holistic care
  • Participate in team meetings, case conferences, and relevant training
  • Document interactions, assessments and support plans per guidelines
  • Keep outcomes and feedback records up to date
