About the Role

Tasks include but are not limited to:

  • Building a service which is responsive to how people we support want to progress
  • Researching new opportunities for community placements and activities
  • Forming and maintaining good relationships with staff at community venues and putting agreements in place to have clear evidence of roles and responsibilities
  • Risk assessing venues and putting in place control measures required
  • Assessing a person’s needs and wishes with a view to offering community options, involving their circle of support where appropriate
  • Supervision of staff
  • Completing Provider-Led care reviews
  • Being able to act as a lead in the day service, this role is equal in grade to a day service deputy manager and the person will be expected to fully integrate with the workings of the service
  • Modelling excellent support practice, being a positive presence in any venue we are operating the service
  • Seeking advice, reporting incidents and escalating concerns when necessary
  • Creating support plans and risk assessments and reviewing these with support staff
  • Competency checks and audits
  • Induction of staff
  • Onward referrals for specialist support
  • Safeguarding referrals and participation in meetings
  • Managing and responding to emergency situations as required

About You

  • Experience of supporting people to achieve outcomes
  • Experience of leadership in care, education or employment
  • An excellent work ethic and positive attitude
  • Transferable skills
  • The ability to work in a physically demanding role
  • A Level 3 diploma in Health & Social Care or a close equivalent qualification.
  • A passion for delivering quality, person centred support.
