The role of Discharge Coordinator is an integral part of the MDT, providing support and specialist advice to the clinical nursing team, AHPs, medical teams, and extended MDT, including social care and outside agencies. You will be expected to provide the highest level of support and advice to enable the timely discharge of vulnerable and premature infants to home and to the care of their family/carers, evaluating our services.

The post holder will use their experience and knowledge to support the transition from hospital to home, working closely with community partners to ensure a safe and supportive environment with ongoing support where necessary.

You will also be expected to actively contribute to multi-professional and multi-agency meetings, providing reports, information, and updates to relevant parties, including safeguarding meetings. There is a link role between the safeguarding team and the neonatal unit. Safeguarding updates and pathways already in place within the trust will need to be embedded in daily practice, with a focus on pathways particular to the neonatal ward to be strengthened. The post holder will lead on quality and safety initiatives, including setting and monitoring standards under the guidance of the Matron.

We have an opportunity for a Band 7 Neonatal Discharge Coordinator at 0.5 WTE in Neonatal ICU, Royal London Hospital, to join our dynamic and friendly team. We are looking for a motivated, ambitious, and experienced Neonatal Nurse to support and work closely with the Neonatal team, partnering with families to ensure seamless transition of care from hospital to community by liaising with the Neonatal Community Outreach team and relevant agencies within and outside the organization.
