About the Post: The Case Worker post will be responsible for undertaking assessments of households in their own homes, working with residents and other stakeholders to implement actions to enable people to remain living independently within their own homes or re-housing to more suitable accommodation where appropriate. This will involve case management of low-level support provision to enable independence at home and facilitate hospital discharges where required. This role will provide a triage function ensuring that referrals and pathways are appropriate to the presenting level of needs and risk, working in partnership with a range of organisations and groups across the borough to achieve the best possible outcomes for individuals, families, and communities.
You will need to demonstrate that you are self-motivated, a good team player, computer literate, have strong written and verbal communication skills, and possess excellent customer-focused qualities. You will also need to collect, monitor and analyse data, so that it can be reported to the Management Team when requested.
The ideal candidate will be able to evidence an awareness of the welfare benefits system, tax credit system, and other sources of financial and practical support, including assistive living technology, minor works, handyperson service, and additional sources of funding such as registered charities.
