This is an exciting opportunity to join a well-established and growing frailty service at Barnsley Hospital, with a strong emphasis on Frailty Same Day Emergency Care (SDEC), multidisciplinary working, and integrated care across the Barnsley place and wider Integrated Care System. This role is particularly suited to those who enjoy front door frailty, ambulatory care and MDT working.

Main duties of the job

The successful candidate will contribute to delivering high-quality frailty care across acute and same-day emergency settings.

Key Responsibilities:

  • Provide consultant-delivered care within Frailty SDEC and front door frailty pathways
  • Undertake Comprehensive Geriatric Assessment (CGA)
  • Support early clinical decision-making and safe discharge planning
  • Contribute to admission avoidance and same-day care pathways
  • Work collaboratively with ED, acute medicine, therapy teams and community partners
  • Participate in the Frailty Virtual Ward and wider frailty pathways
  • Contribute to service development, audit, and quality improvement activity
  • Support teaching and supervision of junior doctors and multidisciplinary staff

Job Plan

The post is based on 10 Programmed Activities (PAs):

  • 8.5 Direct Clinical Care (DCC)
  • 1.5 Supporting Professional Activities (SPA)

Indicative activities include:

  • Frailty SDEC sessions
  • Front door frailty assessment (ED/CDU)
  • Optional inpatient sessions depending on service need and interests
  • SPA time for education, audit, governance and service development

The job plan is flexible and will be agreed with the successful candidate. Those who wish to work part time will be welcomed.

Person Specification

Experience

  • Experience in the management of a broad GIM caseload
  • Experience in the management of an Elderly Care Medicine caseload
  • Experience in a specialist interest that will complement those of other staff

Skills and Attributes

  • Willingness and ability to fulfill the key elements of the job description
  • Possession of the clinical skills required to perform the job including both elderly care and general internal medicine
  • Ability to cope with the demands of a DGH based Consultant
  • Ability to lead and develop the service
  • Ability to develop effective working relationships on an individual, multi-professional and team basis with all levels of staff
  • Ability to communicate effectively, written and orally
  • Ability to empathise and demonstrate probity
  • Ability and evidence of working as part of a multi-disciplinary team
  • Ability to be flexible to meet service demands
  • Understanding of clinical governance and the individual responsibilities it implies

Additional Criteria

  • Audit - Knowledge of multidisciplinary clinical audit principles
  • Teaching - Evidence of effective teaching to medical students and doctors in training and to the multidisciplinary team. Ability to present effectively to an audience using a variety of methods.
  • Fitness to Practice - Occupational health clearance and satisfactory enhanced disclosure from the DBS.
  • Required status to work in UK

Qualifications

  • MRCP or equivalent
  • Full GMC registration, and on the specialist register OR Full GMC registration and within 12 months of specialist registration due to completion of training or within 12 months of specialist registration due to application via CESR with the submission completed at time of application for post
