Barnsley Primary Care Network is recruiting a Care Coordinator to join our multidisciplinary team. You will coordinate activities with PCN practices and MDT colleagues to ensure patients have timely access to services, advice and information.
The role supports personalised, patient-centred care planning, manages a caseload (often with long-term conditions or complex needs), and helps patients engage with services, self-management tools and community resources.
