This role involves working with children who have complex disabilities and their families. Tasks include short break assessments, short break reviews, development of short break packages, transition work for adult services, facilitating events and completing referrals to continuing health care. Some direct family support work with families may also be performed.
What you will need:
- Experience of working with children and their families providing support to improve their situations.
- Experience of working with colleagues across a range of agencies to support children’s outcomes.
- Level 2 qualifications in Maths & English or Level 2 Certificate for the Childhood and Young Peoples Workforce Social Care Pathway.
- ICT training.
